The winger has just over a year remaining on his contract at Stamford Bridge, and Real Madrid are allegedly keen to sign the Belgium international during the summer window.
Hazard could potentially leave on a free transfer at the end of next season, but Chelsea are yet to accept defeat in their efforts to keep the 28-year-old.
According to AS, that may result in Hazard feeling forced to formally request a switch to the Bernabeu rather than see out his deal at the West Londoners.
Chelsea are allegedly demanding in the region of £100m, but Real are likely to do what they can to lower the Blues' asking price.
Hazard has contributed 16 goals and 15 assists from 36 outings in the Premier League this season.
