However, Russell Westbrook and Kevin Durant started to find their feet before the break to leave the sides level at half time, while Kyrie Irving was forced out of the game shortly before the interval with a knee injury.
Serge Ibaka and Anthony Morrow both made three-pointers early in the third quarter as the Thunder started the second half with a 10-point unanswered run to take the lead.
Tristan Thompson tried to get Cleveland back into the game late on, cutting the home side's lead to just four points with two minutes to go, but the Thunder did just enough to hold on with the win.
The Thunder have now won six of their last seven games, leaving them with a 9-13 record this season, while the Cavaliers slip to 13-8 to sit fourth in the Eastern Conference.
