Coach Rui Jorge has left out Ruben Semedo, Kevin Rodriguez and Goncalo Guedes, with the latter pair starting the evening on the bench.
Team captain Tobias Figueiredo returns to take up a place in central defence, while Iuri Medeiros and Bruma add width to the Portugal attack.
Medeiros and Bruma will provide support for advanced forward Goncalo Paciencia, who sits ahead of attacking midfielder Daniel Podence and Bayern Munich's Renato Sanches.
Macedonia have made a change between the sticks as Damjan Siskovski is preferred to Igor Aleksovski, while the prolific Marjan Radeski spearheads their attack.
