Barcelona loanee Clement Lenglet is expected to return to the squad after missing the 2-2 draw at Chelsea with an adductor injury.
However, the Frenchman is more suited to playing on the left-hand side of a back three, meaning that Davinson Sanchez should deputise for Romero.
The remainder of the starting lineup should remain the same providing that Conte does not decide to hand a first start of the campaign to Richarlison.
That appears unlikely due to the form of Dejan Kulusevski, but the Brazilian did make a positive impression off the substitutes' bench in last weekend's London derby.
Along with Romero, midfielder Oliver Skipp is expected to stay sidelined for Spurs as he continues his recovery from a hairline fracture to his heel.
