The 35-year-old, who is the former chairman of the Professional Footballers' Association, was hit by a lorry on the A64 near York last Monday.
According to BBC News, the Metropolitan Police have revealed that Carlisle was charged for failing to provide a sample on December 20.
The retired sportsman, who remains in hospital in Leeds, is due to appear at Highbury Corner Magistrates' Court on January 20.
Carlisle's wife Gemma gave an update on her husband's condition by writing on Twitter: "Clarke still very poorly but we're hopeful he'll make a full recovery."
