The defending champion put his hand to his head in pain before slumping in his chair and disappearing under the table.
Reporters were then asked to leave before the lights were dimmed.
Nadal appeared to be okay minutes later and claimed he was suffering from severe cramp.
The 25-year-old had earlier beaten Dabid Nalbandian in straight sets, but was treated for a foot injury during the match.
