Guus Hiddink has made three changes to the XI that claimed a 4-0 victory over relegation-threatened Aston Villa last time out, with Pato coming in for the injured Loic Remy in his first Premier League start.
Between the sticks, Thibaut Courtois has been dropped in favour of Asmir Begovic, while the other change sees Oscar come in for countryman Kenedy.
Diego Costa remains suspended for the tie, with Eden Hazard, John Terry and Gary Cahill all out injured.
Cahill's absence means another start for young American Matt Miazga at the heart of the Blues' defence.
For the hosting Swans, Francesco Guidolin makes four changes to the side that managed a draw at Stoke City last weekend.
Andre Ayew makes a return to the XI after time out with an ankle injury, while Alberto Paloschi is named ahead of Bafetimbi Gomis up front.
Jefferson Montero comes into the midfield alongside Ayew, Jack Cork, Leroy Fer and Gylfi Sigurdsson.
The other change sees Neil Taylor return to the back four as Kyle Naughton drops to the bench.
