An untidy and fairly uneventful game had looked to be heading for a goalless draw until the Blades were awarded a last-minute penalty.
Facing his former club, Millers defender Craig Morgan brought down Harry Maguire, giving Ben Davies the chance to win the clash from the spot.
Davies did just that, placing the ball into the bottom right corner to give his side their fifth away league victory of the season.
Rotherham and the Blades stay third and 11th respectively in the League One table.
