Scott McTominay and Alexis Sanchez are the two players to retain their places from the goalless draw with West Ham United on Thursday, as Mourinho rotates in preparation for the FA Cup final.
Michael Carrick starts and captains the side for his final league game for the Red Devils before hanging up his boots, although the manager intends to withdraw him before the end of the game to allow the fans of the club he has represented for 12 years to honour him with an ovation.
Daley Blind, Eric Bailly, Sergio Romero, Marcus Rashford, Marcos Rojo, Matteo Darmian, Juan Mata and Ashley Young also come into the side as United cap off their best league finish since the days of Sir Alex Ferguson.
The visitors, meanwhile, show three changes from the side that beat Newcastle United last time out and secured their top-flight status in the process.
